MADWORLDDETOX

The MadWorldDetox Protocol

The Complete Mold Detox Protocol

Mold illness is real. Millions suffer from it. Most doctors miss it entirely. This is the protocol that actually works, based on the Shoemaker method, refined by practitioners who see patients recover. No $10,000 clinic visits required.

Updated: May 2026|6-12 month protocol|30-minute read

Critical Warning: Who Should NOT Do This Alone

Severe CIRS (Chronic Inflammatory Response Syndrome) requires medical supervision. If you have multiple chemical sensitivity, cannot tolerate any supplements, have been sick for years, or score above 8 on the VCS test with multiple elevated inflammatory markers, you need a Shoemaker-certified practitioner.

This protocol is educational. It is NOT medical advice. Mold illness can cause permanent damage if mismanaged. If you have heart issues, are pregnant, on multiple medications, or have severe neurological symptoms, work with a professional.

Protocol Summary

The MadWorldDetox Mold Protocol follows the proven Shoemaker biotoxin pathway: Remove exposure → Bind toxins → Open drainage → Treat colonization → Restore markers. Most people skip steps and wonder why they stay sick.

What Is Mold Illness (And Why Doctors Miss It)

Here is the uncomfortable truth: Approximately 25% of the population has a genetic susceptibility to mold illness. Their immune systems cannot properly identify and eliminate mycotoxins, the toxic compounds mold produces. These people do not get better when they leave the moldy environment. The toxins stay trapped in their bodies, recirculating and causing chronic inflammation.

The other 75% of people can walk into a moldy building and feel nothing. Their bodies tag the toxins, eliminate them, and move on. This is why your spouse, roommate, or family member might be fine while you are falling apart. It is not in your head. It is in your genes.

Mainstream medicine still debates whether mold illness exists. Meanwhile, tens of thousands of people have recovered using the protocols developed by Dr. Ritchie Shoemaker and refined by functional medicine practitioners over the past two decades.

The medical establishment will catch up eventually. You do not have to wait.

CIRS: The Real Diagnosis

CIRS (Chronic Inflammatory Response Syndrome) is the clinical term for biotoxin illness. Mold is the most common cause, but CIRS can also be triggered by Lyme disease, certain algae exposures, and other biotoxins.

CIRS is defined by specific diagnostic criteria:

CIRS Diagnostic Criteria

  1. History of biotoxin exposure, Water-damaged building, tick bite, reef exposure
  2. Symptom cluster, Multiple symptoms across multiple organ systems (see below)
  3. Positive VCS test, Visual Contrast Sensitivity test showing neurological impact
  4. HLA-DR gene susceptibility, Genetic test showing mold-susceptible haplotype
  5. Abnormal biomarkers, Elevated C4a, TGF-beta1, MMP-9, MSH deficiency, etc.

The biotoxin pathway explains why mold illness is so devastating. When toxins enter a susceptible person, the immune system cannot tag them for removal. Instead:

  • Cytokine storm, The body mounts an inflammatory response that never resolves
  • Hormone disruption, MSH (master hormone) crashes, affecting sleep, pain, and mood
  • Leptin resistance, Weight gain, constant hunger, metabolic dysfunction
  • Brain inflammation, Cognitive impairment, memory loss, word-finding difficulty
  • Capillary hypoperfusion, Reduced blood flow causing fatigue and exercise intolerance

This is not a simple allergy. This is systemic inflammation affecting every organ system.

The 37 Symptoms of Mold Illness

Dr. Shoemaker identified 37 symptoms across 13 clusters. Having symptoms in 8 or more clusters strongly suggests CIRS. This is why mold illness mimics so many other conditions, it affects everything.

Neurological

  • • Headaches
  • • Brain fog / confusion
  • • Memory problems
  • • Word-finding difficulty
  • • Concentration issues
  • • Disorientation
  • • Numbness / tingling
  • • Tremors

Respiratory / Sinus

  • • Sinus congestion
  • • Chronic cough
  • • Shortness of breath
  • • Wheezing
  • • Nosebleeds
  • • Sore throat

Musculoskeletal

  • • Joint pain
  • • Muscle aches
  • • Morning stiffness
  • • Cramping
  • • Weakness
  • • Ice-pick pain

General / Systemic

  • • Chronic fatigue
  • • Temperature dysregulation
  • • Night sweats
  • • Static shocks
  • • Excessive thirst
  • • Frequent urination
  • • Appetite swings
  • • Weight gain (or loss)

Eye / Vision

  • • Light sensitivity
  • • Red eyes
  • • Blurred vision
  • • Tearing
  • • Visual contrast loss

Psychological

  • • Anxiety
  • • Depression
  • • Mood swings
  • • Irritability
  • • Insomnia

The pattern to recognize: Multi-system symptoms that started or worsened after moving into a new home, workplace, or after water damage. If doctors keep telling you tests are normal but you feel terrible, mold illness should be on your radar.

Testing: Home and Body

Before starting treatment, you need to know two things: Is your environment contaminated? And is your body showing the markers of CIRS?

Environmental Testing

TestWhat It MeasuresCost
ERMI (Environmental Relative Moldiness Index)DNA-based analysis of 36 mold species from dust sample$290-400
HERTSMI-2Simplified ERMI focusing on 5 most dangerous mold species$150-200
Mycotoxin Plate TestAir sampling for active mold spores$100-150

HERTSMI-2 is the minimum. A score above 10 indicates a problem. Above 15 is dangerous for CIRS patients. ERMI provides more detail but costs more. Test before and after remediation to verify success.

Body Testing

TestWhat It ShowsCost
VCS Test (Visual Contrast Sensitivity)Neurological impact of biotoxins, free online screeningFree-$15
HLA-DR Genetic TestMold susceptibility genes, one-time test$300-400
Mycotoxin Urine TestMeasures mycotoxins being excreted (Great Plains, RealTime Labs)$300-400
CIRS Inflammatory PanelC4a, TGF-beta1, MMP-9, MSH, VIP, VEGF, ADH, osmolality$500-1000
MARCoNS Nasal SwabTests for antibiotic-resistant staph colonization$100-150

Start with the VCS test, it is free and takes 10 minutes. If you fail, pursue HLA-DR testing and the inflammatory panel. The mycotoxin urine test is controversial (some practitioners love it, others question accuracy) but can provide useful data.

Step Zero: Remove Exposure (Non-Negotiable)

No protocol works while you are still being exposed. This is the hardest step and the one people most want to skip. If you are living or working in a moldy environment, every supplement you take is fighting an uphill battle.

The Hard Truth

Some people need to move. Some need to change jobs. Some need to remediate at significant cost. There is no way around this. Binders cannot out-compete ongoing exposure. If your HERTSMI-2 score is above 10 and you have CIRS genetics, that environment is slowly destroying your health.

Options for Addressing Exposure

Option 1: Professional Remediation

Hire a mold remediation company that understands CIRS protocols (not just cosmetic removal). Cost: $5,000-50,000+ depending on extent. Requires HEPA containment, proper removal of materials, and post-remediation testing.

Best for: Homeowners with localized water damage

Option 2: Move

Sometimes moving is cheaper and more effective than remediation. Do not bring contaminated belongings, porous items (mattresses, upholstered furniture, books) often need to be discarded.

Best for: Renters, severe contamination, or when remediation exceeds home value

Option 3: Air Filtration + Mitigation

HEPA + activated carbon air purifiers in every room. IQ Air HealthPro Plus or Austin Air are gold standard. Run 24/7. This buys time but does not solve the root problem.

Best for: Temporary measure while planning remediation or move

Contaminated belongings matter. Mycotoxins embed in porous materials. If you move but bring your moldy mattress, couch, and clothing, you bring the problem with you. Some patients need to start fresh with new belongings. Painful but sometimes necessary. See our guide on environmental toxins for more context on invisible exposures.

The Binders: Your Primary Weapon

Binders are substances that grab toxins in your gut and carry them out before they can be reabsorbed. This is the foundation of mold detox. Without binders, mycotoxins recirculate through enterohepatic circulation, your liver dumps them into bile, they reach your gut, and get reabsorbed back into your bloodstream.

For a deep dive on how binders work, see our complete binder guide.

Prescription Binders (Gold Standard)

Cholestyramine (CSM)

The Shoemaker protocol gold standard. Cholestyramine is a bile acid sequestrant originally designed for cholesterol. It happens to bind mycotoxins with high affinity.

  • Dose: 4g (one packet) four times daily
  • Timing: 30-60 minutes before meals and at bedtime
  • Key rule: Take 1 hour away from all other supplements and medications
  • Cost: $50-150/month (generic, with prescription)

Side effects: Constipation is common. Increase water and magnesium. Can deplete fat-soluble vitamins, supplement A, D, E, K separately.

Welchol (Colesevelam)

Alternative to CSM for those who cannot tolerate it. Less constipating, easier on the gut, but slightly less effective for some mycotoxins.

  • Dose: 625mg tablets, 3-6 tablets twice daily
  • Timing: With meals
  • Cost: $100-300/month (often covered by insurance)

Better tolerated than CSM. Good option for sensitive patients or those who cannot handle the constipation.

Over-the-Counter Binders

If you cannot get a prescription, these OTC binders provide meaningful support:

BinderBest ForDoseCost
Activated CharcoalBroad-spectrum binding, affordable, widely available500-1000mg 2-3x daily$15-25/month
Bentonite ClayAflatoxins, ochratoxin A, heavy metals1 tsp in water 1-2x daily$15-30/month
Zeolite (Clinoptilolite)Heavy metals, ammonia, some mycotoxins1-2g 2x daily$30-50/month
ChlorellaHeavy metals, supports detox pathways3-6g daily$20-40/month
Modified Citrus PectinHeavy metals, gentle, can be taken with food5-15g daily$40-60/month

Critical Binder Rules

  • 1. Away from everything: Take binders 1 hour before or 2 hours after food, supplements, and medications
  • 2. Rotate binders: Different binders bind different mycotoxins, use 2-3 types
  • 3. Stay regular: If constipated, binders trap toxins IN your body, increase magnesium
  • 4. Long-term commitment: Expect 6-12+ months of consistent binder use

Drainage and Liver Support

Binders only work if your elimination pathways are open. If you are constipated, your liver is sluggish, or your lymph is stagnant, toxins have nowhere to go. This is why so many people feel worse on mold protocols, they skip drainage support.

Daily Bowel Movement (Minimum)

You must have 1-2 bowel movements daily. Binders holding toxins that sit in your colon allow reabsorption. Support motility with:

  • Magnesium citrate or oxide: 400-800mg at bedtime
  • Vitamin C: 2-5g daily (to bowel tolerance)
  • Adequate water: Half your body weight in ounces minimum
  • Fiber: Psyllium husk, ground flax, or vegetables

Liver Support

Your liver processes mycotoxins for elimination. Support it with:

  • Milk thistle (silymarin): 200-400mg 2-3x daily
  • NAC (N-Acetyl Cysteine): 600-1200mg daily, glutathione precursor
  • Liposomal glutathione: 250-500mg daily (expensive but effective)
  • B vitamins: Especially B6, B12, folate for methylation
  • Castor oil packs: Over liver 3-4x weekly

See our complete liver support protocol for detailed guidance.

Lymphatic Support

Your lymphatic system has no pump, it relies on movement:

  • Dry brushing: Before shower, brush toward heart
  • Rebounding: Mini trampoline 10-15 minutes daily
  • Walking: 30+ minutes daily
  • Gua sha: Face and neck lymphatic drainage
  • Infrared sauna: 20-40 minutes 3-5x weekly (if tolerated)

MARCoNS and Nasal Treatment

MARCoNS (Multiple Antibiotic Resistant Coagulase Negative Staphylococci) is a deep nasal infection found in 80% of CIRS patients. These antibiotic-resistant bacteria form biofilms in your sinuses and produce exotoxins that keep MSH suppressed.

If you have MARCoNS, you will not fully recover until it is eradicated. The bacteria continuously undermine your hormone system and immune function.

Testing for MARCoNS

A deep nasal swab (API-Staph test) from a lab like Microbiology DX. Cost: $85-150. The swab must go deep, surface swabs miss it.

Treatment Options

BEG Spray (Prescription)

The gold standard. Contains Bactroban (mupirocin), EDTA (biofilm disruptor), and Gentamicin. Compounded by specialty pharmacies.

  • Dose: 2 sprays per nostril 3x daily
  • Duration: 30-60 days typically
  • Cost: $80-150/bottle (lasts ~30 days)

Colloidal Silver Nasal Spray (OTC Alternative)

For those without access to BEG. Less studied but many report success.

  • Dose: 2-3 sprays per nostril 2-3x daily
  • Duration: 30-90 days
  • Cost: $20-40/bottle

Xlear or Saline + Xylitol

Xylitol disrupts bacterial biofilms. Use as maintenance after clearing MARCoNS.

  • Dose: 2 sprays per nostril as needed
  • Cost: $10-15/bottle

Retest after treatment. MARCoNS can return, especially if you are re-exposed to mold. Many patients do maintenance nasal treatment indefinitely.

Biofilm Busters

Biofilms are protective matrices that bacteria, fungi, and parasites create to hide from your immune system and antibiotics. Both mold colonies and MARCoNS create biofilms. Until you break through these biofilms, treatment cannot reach the organisms.

Biofilm Disruptors

  • NAC (N-Acetyl Cysteine): 600-1200mg 2x daily. Breaks disulfide bonds in biofilm matrix. Also supports glutathione.
  • Serrapeptase: 120,000 SPU 1-2x daily on empty stomach. Proteolytic enzyme that digests biofilm proteins.
  • Nattokinase: 2,000 FU 1-2x daily on empty stomach. Another proteolytic enzyme, also supports circulation.
  • Lumbrokinase: Strongest fibrinolytic enzyme. Use for stubborn biofilms.
  • Interfase Plus: Combination enzyme formula designed specifically for biofilm disruption.

Biofilm Protocol Timing

Take biofilm disruptors on an empty stomach, 30-60 minutes BEFORE antimicrobial herbs or nasal spray. The disruptors break open the biofilm, then the antimicrobials can reach the organisms inside. Order matters.

Timeline and Expectations

Mold illness recovery is measured in months and years, not weeks. Set realistic expectations. This is not a 30-day cleanse.

Month 1-2: Foundation

Remove exposure, start binders, establish drainage. Possible Herxheimer reactions as toxins mobilize. Many feel worse before better. Stay the course.

Month 3-4: Stabilization

Energy starting to return. Brain fog lifting. Sleep improving. Inflammatory markers beginning to normalize. Address MARCoNS if present.

Month 6-9: Momentum

Significant symptom improvement. VCS test passing. Most people feel 50-70% better at this stage. Continuing binder protocol.

Month 12+: Restoration

Inflammatory markers normalizing. MSH and VIP restoring (if working with practitioner). 80-100% recovery for most. Some need 18-24 months for full recovery.

Severe CIRS Cases

If you have been sick for 5+ years, have multiple chemical sensitivity, cannot tolerate any supplements, or have severe neurological symptoms, expect 2-3 years minimum. Work with a Shoemaker-certified practitioner. These cases require precise sequencing of interventions and lab-guided treatment.
