The MadWorldDetox Protocol
The 4R Gut Reset Protocol
Remove. Replace. Reinoculate. Repair. The framework functional medicine practitioners have used for decades to heal leaky gut, SIBO, candida, and chronic digestive issues.
Why Gut Health Is Everything
Your gut isn't just where you digest food. It's where 70% of your immune system lives. Where 90% of your serotonin is made. Where the bacteria outnumber your human cells 10 to 1.
When your gut is damaged, by antibiotics, processed food, stress, or chronic infections, the effects cascade everywhere: brain fog, fatigue, skin issues, autoimmunity, mood disorders, weight gain, food sensitivities.
The 4R protocol isn't a quick fix. It's systematic repair. Remove what's causing damage, replace what's missing, restore the ecosystem, repair the lining. In that order.
The 4R Framework
Remove (Weeks 1-3)
You can't heal in the same environment that made you sick. First, remove:
Foods to Eliminate
- • Gluten (all forms)
- • Dairy (except ghee)
- • Refined sugar
- • Alcohol
- • Processed/packaged foods
- • Seed oils
- • Known personal triggers
Pathogens to Address
- • SIBO (if present)
- • Candida overgrowth
- • Parasites
- • H. pylori
- • Pathogenic bacteria
Consider testing (GI-MAP) to identify specific issues
Replace (Weeks 2-6)
Most people with gut issues are missing key digestive factors. Replace what's depleted:
Stomach Acid
Betaine HCl with meals
Low stomach acid is epidemic. Signs: bloating after meals, undigested food in stool, acid reflux (yes, reflux is often LOW acid). Start with 1 capsule, increase until warmth.
Digestive Enzymes
Broad-spectrum, with meals
Protease, lipase, amylase at minimum. Look for full-spectrum enzymes including DPP-IV (helps with gluten/dairy cross-contamination).
Bile Support
Ox bile or TUDCA
If you've had your gallbladder removed or struggle with fat digestion. Bile is also antimicrobial, helps control SIBO.
Reinoculate (Weeks 4-10)
Your microbiome was decimated by antibiotics, stress, and processed food. Time to rebuild:
Probiotics
- Lactobacillus/Bifidobacterium blend: 50+ billion CFU, multiple strains
- Saccharomyces boulardii: Especially after antibiotics or with diarrhea
- Soil-based organisms: Bacillus strains for diversity
Note: If you have SIBO, go slow with probiotics, some can worsen symptoms initially
Prebiotics
- Food sources: Garlic, onion, leeks, asparagus, artichoke
- Resistant starch: Cooked and cooled potatoes/rice
- Supplements: Partially hydrolyzed guar gum, acacia fiber
Start low and slow, too much prebiotic too fast causes bloating
Repair (Weeks 6-12)
Now that you've cleaned house and restored the ecosystem, repair the gut lining itself:
L-Glutamine
5-10g daily
The primary fuel for intestinal cells. Start with 2.5g twice daily, work up to 5g twice daily. Take on empty stomach.
Zinc Carnosine
75-150mg daily
Clinically proven to heal ulcers and repair mucosal lining. Works synergistically with L-glutamine.
Collagen/Bone Broth
Daily
Provides glycine and proline for gut lining repair. Bone broth is ideal; collagen peptides are a convenient alternative.
Additional Support
As needed
DGL licorice, slippery elm, marshmallow root, aloe vera, all support mucosal healing. Especially helpful for acid reflux.
Week-by-Week Overview
| Week | Phase | Focus |
|---|---|---|
| 1-2 | Remove | Eliminate trigger foods, start antimicrobials if needed |
| 2-3 | Remove + Replace | Introduce digestive enzymes, HCl if needed |
| 4-5 | Replace + Reinoculate | Begin probiotics slowly, continue enzyme support |
| 6-8 | Reinoculate + Repair | Add L-glutamine, zinc carnosine, prebiotics |
| 9-12 | Repair | Continue repair nutrients, slowly reintroduce foods |
What to Expect

Recommended Supplies
Quality matters. These are the core supplements for the 4R protocol, sourced from trusted brands.
Spore-Based Probiotics
Soil-based organisms that survive stomach acid. Better colonization than standard probiotics.
Digestive Enzymes
Full-spectrum with protease, lipase, amylase. Look for DPP-IV for gluten/dairy support.
L-Glutamine Powder
Primary fuel for gut cells. Powder form for higher doses. 5-10g daily.
Bone Broth Powder
Collagen, glycine, proline for gut lining. Convenient alternative to homemade broth.
Slippery Elm Capsules
Demulcent herb that coats and soothes the gut lining. Traditional mucosal support.
Marshmallow Root
Mucilaginous herb for gut lining repair. Works synergistically with slippery elm.
Zinc Carnosine
Clinically proven for ulcers and mucosal repair. Works synergistically with L-glutamine.
Apple Cider Vinegar
Raw, with "the mother". Supports stomach acid and digestion. 1 tbsp before meals.
