What It Is

Alpha-GPC is a phospholipid metabolite — specifically, the deacylated remnant of phosphatidylcholine after the body has stripped off its fatty acid tails. It is present in small amounts in dairy and organ meats and at higher concentrations in breast milk, where it supports the rapid brain growth of the infant. Supplemental alpha-GPC is manufactured by enzymatic deacylation of soy or sunflower lecithin.

The molecule is approved as a prescription drug for cognitive decline and stroke recovery in Italy, Russia, and Korea (trade names Gliatilin, Delecit). In the United States and EU it is sold as a nutraceutical. The Italian ASCOMALVA trial (2013-2017) demonstrated meaningful preservation of cognitive function in Alzheimer's patients adjunctively treated with alpha-GPC plus donepezil versus donepezil alone.

Choline Sources Ranked

Not all choline supplements deliver the same active compound to the brain:

  • Alpha-GPC — ~40% choline by weight, crosses BBB rapidly, directly raises brain acetylcholine. Best for cognition.
  • CDP-choline (citicoline) — ~18% choline; converts to alpha-GPC and cytidine, also raises dopamine. Excellent for stroke and TBI.

How It Works

Alpha-GPC has three downstream destinies in the body — and all three are clinically relevant. In neurons, it donates its choline to ChAT (choline acetyltransferase) for acetylcholine synthesis. In hepatic and neuronal membranes, the glycerophosphocholine backbone is reused for new phosphatidylcholine. And in the pituitary, it modulates somatotrope activity to release growth hormone.

Contraindications & Cautions

  • Stroke risk signal: A 2021 Korean cohort study reported a small but statistically significant association between long-term high-dose alpha-GPC use and stroke incidence in older adults. The mechanism is unclear; conservative recommendation is to limit to therapeutic doses and durations.
  • Depression and low-acetylcholine sensitivity: Some individuals — particularly with low dopaminergic tone — experience depression or apathy on cholinergic loading. Drop dose if mood worsens.
  • Bipolar disorder: Cholinergic loading can shift bipolar mood; introduce cautiously.
  • Anticholinergic medication users: Will partially offset; not a contraindication but reduces effect.
  • Pregnancy / breastfeeding: Choline is critical for fetal brain development; alpha-GPC is present in breast milk. Therapeutic doses lack pregnancy data — stay at the food-equivalent level.
  • TMAO concerns: Like all choline sources, gut conversion to TMAO is possible. Support gut flora with prebiotics; rotate with PC for lower TMAO yield.
  • Sleep disruption: Acetylcholine surge before bed can promote vivid dreams and lighter sleep. Take in morning and afternoon.
